Patients can be admitted to Peterson Rehabilitation Hospital through a referral from the social worker or case manager at the acute hospital, long term acute hospital, skilled nursing facility, long term living facility, hospice, home health agencies, and assisted living facilities. You will need to request that the patient’s information be faxed to Peterson Rehabilitation Hospital’s Admissions Department.

 

We also admit patients through physician office referrals directly from home. You will need to request physician office to fax patient’s information to Peterson Rehabilitation Hospital’s Admissions Department.
